Role: The
Grade D Nurse will be responsible for the assessment of care needs
and the development of programmes of care and or the implementation
and evaluation of these programmes. The post holder is expected
to carry out all relevant forms of care without direct supervision
and may be required to demonstrate procedures to and supervise
registered and non-registerd staff.

Minimum 1 years relevant post registration experience

- Community Nursing Service
Role: The
Grade D Nurse is a member of the Community Nursing Team and is
responsible for carrying out duties within the programme of patient
care and in the patients' homes, doctors surgery, Health Clinic
or Health Centre.

Registered General Nurse on Part 1 or 12 on the live UKCC Register
Minimum 1 years relevant post registration experience
Treatment
Room/Community Nursing experience an advantage Car Driver and

Grade D
- Hospital at Home
Role: The
Hospital at Home Services is available for patients who would
otherwise require hospitalisation. The service will provide a
direct alternative to hospital care, allowing the patient choice
as to whether they are cared for in hospital or at home. The service
provided will be complementary to that of the Primary Health Care
Team and provide an extension oto District Nursing Service for
the more dependent patient.

Community

Grade E
Role: The

Grade E
- District Nursing Service/Treatment Room
Role: The
Grade E Nurse is a member of the Community Nursing Team and is
responsible for the assessment of care needs and the development,
implementation and evaluation of programmes of care to patients
in the community in any setting. Will be required to take charge
in the absence of the sister and supervise junior and unqualified
staff.
Criteria:
Registed General Nurse on parts 1 or 12 on the UKCC live register
Minimum 2 years relevant post registration experience. Car driver
and access to a car essential Treatment Room/Community Nursing
experience is desirable
Community
Midwife Grade E
Role: The
Grade E is responsible for delivery of midwife services in the
Community. The assessment of midwidery needs and the development
and implementation and evalutation of programmes of care. The
staff midwife works within the UKCC Code of practice for midwives
which is complementary to the Handbook of Midwives Rules. She
will be required to take charge in the absence of the midwifery
sister.
Criteria:
Registered midwife on the UKCC live Register Currently updated
in Refresher Midwifery Course and holding an up to date Intention
to Practice Notificatio.n Car Driver and access to car essential.
Community midwifery experience would be desirable
Grade E
- School Nurse
Role: The
School Nurse as a member of the School Health Team has a responsibility
to provide the School Health Programme to the population within
his/her responsibility.
Criteria: Registered General Nurse on Part 1 or 12 on the live
UKCC Register Minimum 2 years relevant post registration experience.
Car Driver and access to a car. Experience in Paediatric Nursing/Health
Visiting is desirable
Grade
E - Hospital at Home
Role: Hospital
at Home is the provision of intensive levels of care for acutely
ill people in their own homes. This involves bringing a hospital
level of care to the home supplying medical, nursing and rehabilitation
services as well as social work support and equipment. The service
is complementary to that of the Primary Care Team and provides
a support to the District Nursing Services. The Hospital at Home
Grade E Nurse will provide skilled nursing care under the supervision
of the District Nurse. He or she will assist the Hospital at Home
Care Manager in arranging and co-ordinating the packages of care
to patients admitted to Hospital at Home.
Criteria:
Registered General Nurse on part 1 or 12 on live UKCC Register.
Minimum 2 years relevant post registration experience. Car Driver
and access to a car. Paediatric Qualification/Community Experience
